Step 1. Contractor Clicks on the Link for confidentiality agreement: powerforms.docusign.net/fb8f0edd-0fa3-4fc6-b599-e2f5f7aef452
Step 2. Please add Equana Galloway, galloway-equana@cooperhealth.edu, the CUHC on the 1st screen of the confidentiality agreement.
This is an extremely important step so that the document is sent to the appropriate person.
Click on Send Form another screen will open.
Step 3. Once screen open, click on continue. Please fill out all required fields. You will need to enter a HIPAA Training and Cybersecurity Training on questions 4 and 5. HIPAA & Cybersecurity training has to be within the last year. Your 4-digit identifier is a personal pin you can use when you call the Cooper help-desk to reset your Active Directory password.
Step 4: Hit the Finish Button at the bottom of the page or on the red banner to send the form back to Cooper contact.
Step 5: Docusign Form will be automatically sent back to the Cooper Contact
Step 6: The Contractor (student or preceptor) will have the option to Download, Print, Close after clicking Finish.
Once the Cooper contract signs off on the confidentiality agreement, the contractor will be alerted.
Step 7: Our Identity and Access Management IT Team will create your AD and Cooper email.
Step 8: Once you receive your Cooper AD email and password please log into our Citrix portal to activate your AD account
Step 9: Enter this URL - https://remote.cooperhealth.edu
Step 10: You will be prompted to change your password once you login for the 1st time.
