Cooper Connect was created to help patients and the community easily access their health-related information anytime and anywhere. As technology rapidly advances, Cooper aims to ensure people have the tools to stay connected to these resources. With the growth of health apps, patient portals, and other digital tools, patients can now interact with their health care teams in new ways, improving their experience, supporting their health outside of clinical visits, and enhancing overall health outcomes. Cooper Connect is here to help support this process.
We do this by helping patients to learn and access MyCooper (MyChart) and Cooper’s social determinant of health resource tool, CooperUnite.
Cooper Connect team members engage the community so they can:
- Understand digital tools
- Communicate with their doctors
- Access test results
- Request prescription refills
- Keep track of upcoming appointments
- Search free and low-cost community resources
