Community Health Workers

About the Program

Community Health Workers (CHWs) provide peer-to-peer support to people experiencing social barriers that impact health outcomes. The CHWs come from the communities they serve, are knowledgeable about the system barriers patients face and use their shared background and life experiences to help address barriers to care. 

How It Works

Individuals are partnered with a Community Health Worker to:

  • Find transportation, healthy foods, employment, medication assistance and more
  • Plan, set, and achieve personal goals
  • Find community resources
  • Understand your health insurance benefits
  • Improve self-support and advocacy skills
  • Improve your overall health and well-being
